Job Description :
A facility known as a treatment center, not-for-profit research center and hospital based in Southern CA is ranked number one of the Best Cancer Hospitals by US News & World Report for over a decade is currently looking to fill (2) Care Coordination RNs to communicate and facilitate patient care along the continuum through effective resource coordination as a patient advocate and may serve as a preceptor and mentoring to other unit/dept activities. Evaluates the patient's progress and communication of the plan of care.
Duration: 13 weeks
Shift: 40 hrs/wk 8:30am to 5pm (w/rotating weekends)

Required Qualifications :
*Current CA RN license
*BLS, issued by the American Heart Assn
*Bachelor's Degree in Nursing
*ACM/CCM certification
*Two yrs case management experience preferred
*Three yrs current clinical experience in area of expertise preferred
