Job Description :
Responsibilities include:
- Direct patient assessments, care management, and coordination of services
- Identifies and links patients to appropriate clinical and non-clinical resources, including social and behavioral
- Provides support to patients newly diagnosed and recently hospitalized to sustain clinical stabilization
- Strong focus on patients with congestive heart failure
- Assists with consultation and counseling of patients, caregivers, and their families
- Supports clinical interventions to assist patients in regaining optimal functioning
- Monitors effectiveness of interventions and communicates changes in plan of care
- Active member of the cardiovascular interdisciplinary team

Required Qualifications :
Qualifications:
- Be both innovative and passionate about improving patient care
- Current California RN license
- Minimum 3-5 years experience in the healthcare industry
- Experienced with cardiovascular and CHF-specific care
- Experience delivering care in a clinic setting
- Ability to work independently in the field.
- Proficient with Microsoft Office products
- Excellent verbal and written communication skills
