RN Case Manager needed in the Santa Clara area
Home Health Case Manager needed
Qualifications:
CA RN License in good standing
Minimum of 1 year Home Health Case Management experience
Current CPR
Licensed driver with an automobile that is insured in accordance with CA state/or organization requirements and is in good working order.
Demonstrates excellent observation, problem-solving, verbal and written communication skills; nursing skills per competency checklist.
Self-directed and able to work with minimal supervision.
Monday through Friday with 1 weekend per month
Salary range is $90,000-$110,000 per year
